Your First Appointment  


Your first appointment with us will most likely be as a referral from another medical doctor.  Your referring doctor will complete a consult request form which his/her office will send us prior to your appointment or which you will bring on your day of appointment.  This appointment will involve a comprehensive eye evaluation, including a detailed examination of your vitreous and retina.  You will need to complete the Patient Demographic Collection Form, the Review of Systems Medical History and an Acknowledgement for the Notice of Privacy Practices prior to the eye examination.  The Notice on Health Information to Patients may be downloaded from this website as well.  You may bring these completed forms on your appointment day or you may complete these in the office while waiting to be called.  Your eyes will be dilated and your vision may become blurred for several hours.  Please set aside at least two hours for your initial consultation.  If additional tests are required, your visit could be prolonged.  You may wish to have someone with you to help drive you home.
