For our patients with dual Medicare-Medi-Cal (Medi-Medi) insurance

We would like to inform you of changes in Medicare and Medi-Cal that have the potential to impact your choice of physician and could result in you being re-assigned to another doctor without your permission.

Beginning now and with full force in 2013, you will be receiving a letter from Department of Health Care Services (DHSC) that will ask you to choose a managed health plan.  You must indicate on that form that you wish to maintain Regular Medi-Cal (option 000).  By doing this, you will maintain your continuity of care and can continue to receive the highest quality of care at Retina-Vitreous Associates.

Please note that if you do not respond to the DHSC letter and indicate a choice, you will AUTOMATICALLY be assigned to a managed health plan network and a new network physician and you will no longer be able to see physicians at Retina-Vitreous Associates.  As a result, if you wish to continue to see us you must respond to the HSC letter and indicate that you want to remain in Regular Medi-Cal plan (option 000).
